If your student has a food "preference/intolerance" (eg. Vegetarian or lactose intolerance etc.) or a diagnosed allergy please contact your student's school nurse. For a preference/intolerance please complete a Dietary Preference Form. For a diagnosed allergy, a medical statement or Medication Order completed by a licensed medical professional will be required as part of your student's health record. Non Recognized FDA Food Allergens
D51 has received requests for ingredient lists to ascertain which menu items contain dyes, annatto, caramel color, paprika, corn syrup and others. All of which are not currently identified by the FDA (Food & Drug Administration) as Common Food Allergens..
D51 and menu software providers will not list or identify the above items due to frequent product manufacturer and formulation changes. D51 will provide menu software which readily identifies the FDA common food allergens.
D51 will create a specific menu for non FDA Common Food Allergens cited above for student(s) with a signed request by the attending physician.
